Additional Named Insured Form
Insures your play and practice facilities. A certificate will be issued to the covered facility once the Commissioner’s office receives the application. Fax directly to the Association office (805.462.1026) Download

Player Medical Waiver/Affidavit/Release Form
This is a combination form and is the only form recognized by CCASA. It includes team information, a medical waiver signed by the parent and a player agreement signed by the player and the player’s parent. ALL parties must read this form in full. To be enforceable, this form, minus the release signature, must be in the possession of Jeff Dean. Download

Individual Player Registration Form (Winter Registration Only)
This form is used to register players not registered ASA in the current year to participate in a Winter softball program. Please fully complete the registration form a mail to the address provided on the registration form. Download

New Team Registration Form (Winter Registration Only)
This form is used to register a team that was not registered ASA in the current year. Please fully complete the registration form a mail to the address provided on the registration form. Download
